Club Charter Application

This form is to be completed for all new and returning clubs in order to be officially recognized by Dominican College. In addition to this form, each club must have a Club Advisor Agreement on file for each academic year. This form must be completed by the club's advisor.

  • Please refer to the online club manual for any questions regarding requirements for clubs & organizations in order to keep an active status. You may also reach out to the Director of Student Activities with any questions.

  • Club Officer Information

    Please make sure that all proposed officers meet the cumulative GPA requirement of 2.25 or higher.

  • Membership

  • At least 5 members must be degree-seeking full-time or part-time students at DC (club officers are considered the first four members of the list). Include ID numbers, phone numbers, and DC email addresses.
