A Closer Look at the NYS Social Care Networks Initiative

On Friday, January 31, 2025, Dominican University hosted an informative presentation on New York’s Social Care Networks (SCN) Program, a transformative initiative under the Medicaid Redesign Team (MRT) and the NY 1115 Waiver.

Ron Colavito, of Access: Supports for Living and a board member of the Hudson Valley Care Coalition, led the discussion, offering valuable insights into the development and statewide impact of Social Care Networks. Attendees gained a deeper understanding of how SCNs are reshaping services for Hudson Valley residents and beyond, as well as explored funding opportunities within the SCN framework.

The presentation also highlighted the critical role of assessments and data collection, emphasizing their importance in improving service coordination and generating actionable insights. Colavito provided a candid assessment of what aspects of social care delivery are working effectively and where challenges remain at a larger scale.
