Submitting Vaccination Records

Vaccination records are maintained by the Student Health Center. As per Public Health Laws §2165 and §2167, all students must provide documentation for immunizations from measles, mumps, and rubella (MMR). Resident student and student-athletes must also provide documentation for immunizations meningitis. A signed waiver to decline vaccines is also acceptable.

These records should be submitted online, through the Dominican University Health Services Patient Portal (Medicat).  All students must be in compliance prior to the first day of classResident students must be in compliance before moving into the residence halls.

To submit your vaccination records, please follow the instructions below:

  1. Visit MY.DC.EDU and click the link to Medicat Patient Portal.
  2. Follow the directions to register. New users must enter first and last name, DC email address, and DC student ID number. You will also be asked to create a username. Once you click “submit” you will receive an email in your Dominican University account, asking you to set up your password.
  3. Login using your new username and password. Your name should appear in upper right-hand corner.
  4. Complete all items on “To Do List” on the HOME screen (scan and upload immunization records, sign meningitis waiver (if applicable), input vaccination dates).
